Membership
We offer two
types of
one-year
memberships that
can be accessed
on-line: Regular
Membership and Organizational
Membership.
Regular
Membership ($50)
– This includes
free evening
programs,
reduced rates to
half-day
programs,
notices of
special events,
and access to
website
resources which
include job
postings, the OD
consultant
locator, program
materials, tools
and resources.
Organizational
Membership ($45
per member)
– This includes
all the benefits
of regular
membership if an
organization has
three or more
members, and
pays for all
memberships with
one transaction
(one check or
credit card
payment). To
receive the $5
membership
discount per
member please
complete a
membership
application for
each person and
mail or fax them
to AzODN with
full payment.
Additional
Listing Option
Consultant
Locator Listing
($25) – This
option is
available for
members who
would like to be
listed as
consultants on
our website.
Your biography,
company services
and link to your
website will be
posted. This
option is
available on our
website, select
“Consultant
Locator Listing”
from the drop
down menu and
submit your
payment and
e-mail your
information in a
Microsoft Word
Document (limit
of one page) to:
info@azodn.org.
You may signup online or you may
print
and fax or mail your application
to the AZODN office with full payment.
Payment
Options
The payment
system is off
line which means
you will be
asked to send in
your payment via
check or credit
card to our
mailing address.
Once you
complete the
membership form
you will receive
an e-mail
immediately to
verify your
information has
been received;
once your
payment has been
processed, you
will receive an
e-mail
confirming your
membership and
your password.
When your
membership is
nearing
expiration the
system will
automatically
send you a
notice.
Check:

we request that
you please pay
by check. Please
make your check
payable to AzODN
and send it with
this completed
form to 3370 N.

85251. Your
canceled check
is your receipt.
Credit Card:
We accept Master
Card and Visa.
If you are
paying by credit
card, please fax
your membership
form(s) to our
fax number
(480-874-9929)
along with
credit card
information
which is
outlined on the
membership form.
The fax is
received
directly by our
treasurer
ensuring the
confidentiality
of the
information.
